Full Time Case Manager RN at Oro Valley Hospital 10 am to 8 pm (10 hours/4 days ) working in the Emergency dept.
Schedule is varying week days and some weekend shifts in Case Management
Candidates need acute hospital experience, prior case management experience preferred.

The Care Manager - RN is responsible for coordinating and overseeing discharge planning, transitions of care, and case management activities to ensure optimal patient outcomes. This role involves collaborating with interdisciplinary teams, reviewing medical records for appropriateness and medical necessity, and maintaining compliance with federal, state, and accreditation standards. Assists in development, planning, coordination and administration of the activities of clinical review, discharge planning, resource utilization and utilization review.
