Job Description

Under the direct supervision of nursing and medical staff, provides safe and therapeutic care either directly or indirectly for an assigned group of patients. These activities may include all functions within the scope of practice in Tennova Health Care.

High School diploma or GED required. 

1. Seeks individualized patient information/instructions and goals from
licensed nursing staff and performs care within scope of practice.
2. Assists with reinforcing patient specific plans of care and hourly rounding.
3. Vital Signs, Patient weights, Intake and Output as directed.
4. Encourages patient Incentive Spirometer use if ordered.
5. Obtains specimens as directed by nursing staff, per protocol.
6. Participates in Purposeful Rounding.
7. Advanced or additional skills including and not limited to phlebotomy and
EKG’s.
8. Responds to call lights in a timely manner.
9. Assists patients with the use of adaptive devices.
10. Documents daily care of patient per unit standards.
11. Identifies and communicates new patient care issues/concerns.

12. Takes rapid action in life threatening situations and immediately notifies the
licensed nursing personnel.
13. Assists with maintenance of Stock Inventory and Refrigerator Temperature
Logs.
14.Performs direct patient care related to mobility.
a. Assists patients with bed mobility/repositioning, transfers, and
ambulation, as appropriate.
b. Assists patients with use of assistive devices, as directed.
c. Documents per unit standards.
d. Demonstrates the behaviors of safety awareness in all aspects of
daily tasks and work flow.
15. Communicates patient outcomes and changes to other members of
the healthcare team through documentation and/or verbal report per
unit standards.
a. Identifies skin abnormalities while assisting patient with Activities of
Daily Living and Mobility.
b. Identify patient safety concerns while assisting with Activities of Daily
Living and Mobility.
c. Reports any concerns/abnormal findings immediately to nursing
staff.
16.Assists with patient admission and discharge process.
a. Ensures complete room set-up for all new admissions
b. Obtain vital signs upon admission and as directed by nursing staff
and admission protocol.
c. Assists with welcoming patients and family to unit.
d. Assists with physical transportation of patients to/from unit.
e. Asks for assistance to complete workload if needed.
17. Practices safe techniques in all aspects of direct and indirect patient
care delivery.
a. Checks, cleans equipment, materials as appropriate and indicted by
the Infection Control protocol.
b. Demonstrates safe movement of patients. Consistently utilizes
proper body mechanics and available support equipment to ensure
patient’s safety and avoid personal injury.
c. Provides patient with appropriate assistive devices as needed.
d. Transports patients as required.
