Northwest Health is seeking a self-starter that has the management capabilities to lead our team forward. If you join our team in this role, you will lead a team to continue development and implementation of quality and healthcare cost management programs; be the primary leader and liaison between the insurance companies and our member practices to develop and implement our value-based strategy for top tier quality through development of reports and review of data, sharing of best practices and creating an improvement plan to help our member practices be successful in a value-based healthcare model.

Perform ongoing review of CIN data to identify areas of focus and opportunities to improve. Develop quality improvement plans for CIN in coordination with the other CIN staff and Quality Committee. Primary contact to IT for coordination of ongoing metric development, definitions and data sourcing between CIN and providers (eg, parameters such as eligibility, optional exclusions, age, attribution ranking, etc.). Primary resource for questions from physicians and office staff participating in the network (eg, HEDIS, NCQA). Responsible for the initial planning and implementation of the population health platform to CIN network providers including, but not limited to, training CIN members on use of the platform, identifying and assisting providers with optimal use and workflow in utilizing the platform and quality data as well. Strong verbal and written communication skills. Strong positive problem solving approach to issues, with a wide degree of creativity. Strong planning skills - track record for accomplishing goals and meeting deadlines. Process improvement oriented, generates new ideas that can positively contribute to success. Motivated self-starter. Transparency-minded. Collaboration and teamwork-oriented. Able to prioritize work and manage time. Able to work flexible hours for early and late meetings.
