Designs and implements Diabetes education program designed to increase awareness and knowledge & to be a resource for providers and patients in the community. Responsible for directing & coordinating all diabetes management care based on established clinical nursing practices. Keeps up-to-date on change in health care technology to keep the program material current. Collaborates with other professional disciplines and follows ADA guidelines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Associate degree in Nursing required, Bachelor's degree preferred. At least 2 years of experience in the field or in a related area.
Responsible for directing & coordinating the diabetes education program based on established clinical practice guidelines. Primarily works in an outpatient setting but is responsible for assisting with inpatient diabetic education as needed.
Promotes patients' independence by establishing patient goals and teaching patients and families to understand conditions, medications, and self-care skills. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks, within the scope of a licensed RN. A wide degree of creativity and latitude is expected. Reports to a manager or CNO. Performs other duties as assigned.
