About the Job:
The Swing Bed Coordinator is a professional nurse who is accountable to the coordination and facilitation of quality clinical nursing for the swing bed patients. Job requires an extremely high level of independent judgment. This role actively participates in a collaborative, consultative, and supportive role with all levels of care providers and other members of the interdisciplinary team to mutually plan, develop, implement, and evaluate:
1) Quality resident care; 2) Competency based nursing care; 3) Collaboration with the MDS Coordinator to meet State and Federal resident care regulations; 4) Level of care changes; 5) Unit and/or institutional programs/projects; and 6) Ongoing education activities.
