Behavioral Health Placement Specialist - PRN

The Transfer Center at Community Health Systems is seeking experienced medical and behavioral health case managers (non-degreed) to work days or nights within our telehealth triage team. Placement Specialists will be responsible for assisting clinicians in the placement of behavioral health patients. Our placement team works collaboratively to improve the flow of behavioral health patients from ER and inpatient settings into programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

The Transfer Center at Community Health Systems is seeking experienced medical and behavioral health case managers (non-degreed) to work days or nights within our telehealth triage team. Placement Specialists will be responsible for assisting clinicians in the placement of behavioral health patients. Our placement team works collaboratively to improve the flow of behavioral health patients from ER and inpatient settings into programs, leveraging intake processes and telemedicine modalities to expedite the access to definitive behavioral health care.

Examples of the work our Placement Specialists do day-to-day;

Manage a high volume of calls from ER and medical floors within the CHS network from clinicians to place patients within the proper level of care.  Hold responsibility for monitoring flow of patient care from facilities to psychiatric units.  

Demonstrates knowledge of regulatory requirements such as EMTALA, CHS Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives. 

About this team

The Behavioral Health Transfer Center at Community Health Systems is a state of the art clinical assessment and patient placement hub.  Composed of Registered Nurses (RN), Social Workers, Therapists, and Providers, this team works collaboratively to provide behavioral health assessments and referral/placement services for patients located throughout CHS hospital system and affiliated providers who may benefit from inpatient and outpatient behavioral health services.

Qualified applicants will have:.

• Higher education at some level in a behavioral health or emergency medical field is highly preferred
• Experience in behavioral health case management or other healthcare field preferred.
• The ideal candidate will also have the ability to utilize various means of technology, and multiple systems at one time to ensure the needs of the patients are met accordingly.
