Job Summary

Using discretion, independent judgment and clinical problem solving, the Case Manager serves to provide management of the patient's hospitalization from pre-admission through post-discharge payment.  The purpose of Case Management is to have a multidisciplinary, collaborative team approach to managing all patients in the most efficient and effective manner.  This individual will possess the skills to conduct psychosocial interviews, negotiation and conflict resolution, and appropriate chart documentation.  He/She is an effective communicator and manager of information in coordinating health care decisions.  This individual will be knowledgeable of the medical aspects of acute and chronic illness treatment and care options and to implement the appropriate psychosocial interventions as needed.  The Case Manager will oversee effective service delivery and utilize managed care concepts in insurance relations, fiscal processing, and completion of precertification.  The Case Manager will assure that healthcare ethics and regulations pertaining to patient's rights are fulfilled.  This individual will provide patient/family advocacy, admission, and discharge coordination, communications related to patient care, conferencing, coordination of community resources and support systems.  This individual will assist to provide information on public benefit programs, compensation, work-related issues, life planning, environmental adaptation, and assistive technology.  The age of the patients served by this facility range from neonate to geriatric.  The individual in this position will possess an understanding of age dynamics, multicultural psychiatric and behavioral concepts.  The team member in this position provides direct case management services but does not provide direct patient care.

Minimal Acceptable Job Qualifications

Licensure:       Current Indiana State RN license.

  • BCLS within 30 days of hire
  • SAMA Level 1 within 30 days of hire 

Experience:    Five years experience in a hospital setting as a Registered Nurse or two years experience in hospital case management/utilization review. 

Education:      Completion of nursing program and/or Bachelor degree.

Other:               Demonstrate knowledge of the hospital service theme. 

                           Demonstrates a working knowledge of insurance industry requirements. 

                           Demonstrates strong interpersonal and communication skills.

                           Must be able to accept responsibility, make independent decisions and interact with professionals.

                           Must have the ability to manage multiple tasks/projects simultaneously.
