Job Description

Responsible for assessing and reassessing patients, maintaining accurate patient records including filing of reports in designated areas, scheduling of appropriate tests when indicated, responding to patient phone calls, speaking with pharmacies when indicated under the supervision of the provider.

Education: 

  • Graduate of a high school or equivalent required.
  • Graduate of an accredited LPN School of Nursing Program required.

 

Certifications/Experience:  

  • Current Indiana License required.
  • Current CPR Certification preferred, required within 3 months of hire.
  • Provide direct patient care, evaluates interventions and consults with provider as necessary to ensure optimal patient outcomes.
  • Perform triage of patients per protocols established by the physician and/or organization.
  • Prepare patient for provider and assist provider with patient examination, procedures and other processes.
  • Provide patient education or coordinates the provision of patient educational activities.
  • Collect patient health information which is communicated written and verbally to appropriate health professionals in order to identify actual and potential problems.
  • Arrange consults, tests, and procedures and verify orders for patients.
  • Demonstrate an ability of safe medication and treatment administration.
  • Correctly identify patients accepting responsibility for individual actions including activities that may be considered fraudulent, illegal or inappropriate.
  • Adhere to organization’s laboratory quality control policies, document all quality control activities, instrument and procedural calibrations and maintenance performed. Deliver care in a nonjudgmental, nondiscriminatory, and ethical manner which preserves and protects the patient autonomy, confidentiality, dignity and rights.

  • Team Member is accountable for areas of responsibility meeting regulatory requirements including but not limited to maintaining annual TB testing, completing annual mandatories (ALC and competency), and maintaining current registration/certification/licensures as appropriate to position requirements.
